Dear Walden PDK Chapter Members,

My name is Karine Clay and I’m the newly elected Foundation Representative for Phi Delta Kappa online Walden University Chapter. I am currently working on scholarship fundraising opportunities to increase PDK’s fund-raising potential. Step 1: Visit the Melaleuca Website

Step2: Become a Customer
To enroll as a Melaleuca customer, the one time membership cost is $29.00 plus state tax. To receive the products at wholesale, each family membership agrees to purchase a low monthly minimum of 35 product points (about $45.00 - $55.00).
Walden PDK chapter will receive 5% to 7% of the purchase price every month you shop towards our Scholarship Fund.
Fill out all the required fields of the customer agreement form (pdf document, 209K)..

Step 3: Make Your First Order
All completed applications must be accompanied by a 35 point minimum first order. Please complete the sample order form (pdf document, 220K) and fax along with your application. After your completed application is on file you will receive a confirmation email that you are now a Melaleuca customer. All following monthly orders can then be submitted by you conveniently via telephone (800) 282-3000 or online at http://www.melaleuca.com.
