Please use this form to nominate a school library to be the recipient of our Literacy Project book donation program for 2008. The information you submit here will be placed on a ballot page before the end of this meeting*, where all of our members will be able to select their preference to receive this year's award of books.

Nominated libraries will be listed on the ballot in the order in which the nominations are posted.

*Note: The email addresses and phone numbers will not appear on the ballot, but will be needed if your library is selected for the award, so that information must be included with the nomination.
